    I grabbed a small section from our knowledge base article that can be helpful to your dilemma.

Over time, your honor will decay (and your crests may disappear) if you do not continue to earn Honor throughout your games played on the Fields of Justice. We made Crests temporary because we didn't want players to be able to permanently unlock them and potentially turn to bad behavior. A player who is truly Honorable should never be in the 1.5% of players that are punished by the Tribunal. If you are banned or suspended by the Tribunal, you will lose all of your crests and will have to earn them again once your suspension is up.

    As of February 8th, 2013, we reduced the threshold for unlocking the Great Teamwork, Leader and Mentor crests, making them slightly easier to obtain. At that time, some players were granted new crests, since they qualified under the new criteria.

    We also increased the threshold required to unlock the Honorable Opponent crest, making it more difficult to earn. Players who did not meet this new criteria lost their Honorable Opponent crest, but can regain it by earning enough honor to meet the new requirement.
    Lucky enough all you have to do is be awarded more honor from your fellow players and your ribbon will return. Seeing as you have earned it once, I don't think it should be a problem to achieve again

    You more than likely haven't hit the threshold to obtain the ribbon. The number you see on your profile is actually the number of people that have honored you — not your honor value. Your honor value depends on what sort of players are granting you honor. Being honored by a player you've just met will have greater weight than being honored by a friend you play with all the time or that you have queued together with. Also, if you are being honored by players who grant honor very frequently, their honor will be less valuable than players who save their commendations for only those who are a shining example of the Summoner's Code.

    It sounds like you had multiple ribbons awarded to you. While you decayed down from the Great Leader ribbon, your account now shows the rarest ribbon you have earned(the Green ribbon in this instance). It could be that you have earned other honors but only the rarest of your awards will be shown.

    Sorry for the delay, I was having trouble keeping my eyes open last night and had to crawl into bed.

    The rarity of ribbons is constantly changing depending on how many members in the community have a certain ribbon, though I do not have the data available to determine which ribbon is currently sitting at the top of the list. I took a look into your account and I noticed you have indeed earned a large amount of honor, a vast majority of which are in the Teamwork (Green Ribbon) and Honorable Opponent(Red Ribbon) categories, congrats . Like the other honor ribbons, you only need to break the threshold in the appropriate categories in order to once again obtain the yellow ribbon, though unfortunately not even I know what the thresholds are.

    This sounds like while you were being honored by friends after every match, the collective honor was not enough to keep you above the threshold for either ribbon. This would be an intentional feature of the honor system. When you are honored by friends you receive less honor than if you were to be honored by a person you were playing with for the first time, and if your friends were continually honoring you after each game it would weaken their honor value even more.

    I don't want to steer this discussion in the direction of the Tribunal or about claims of false reports. But I did take a look into your most recent tribunal punishment and I can say it was well deserved. False reporting does not lead to bans by the tribunal and the loss of honors, toxic behavior does.

    Hmm this is the first time I have heard of this happening. I'm not sure what would be the cause of this but I will get in contact with a couple colleagues and see if I can find out what would cause a notification of one award to appear but not to be shown in the loading screen.
